Can someone recommend a good tour guide? I have experienced the Amazon lodges and now am looking for something more adventurous. Are there some tour guides or a tour company in Iquitos that specializes in "extreme tours?"

I have a friend in Iquitos, Jimmy Barrera, He is an experienced guide who speaks english very well. I'm sure He can help you out, We were just there in March and spent a little time with him and his family in Punchana (Iquitos).

Please use care when contracting a guide for an "exteme tour." Few guides will even bring basic first aide equipment such as snake anti-venom. There is an excellent snake anti-venom (suero antiofidico) made by Probiol that is effective against most poisonous snakes found in the Amazon. Recently a tourist went missing and his body was found a month later. The coroner listed death as due to a snakebite. Be safe and be smart. Refuse to partake in an expedition unless the guide provides a basic first kit containing snake anti-venom.
